Please note that the owner of the veterinary clinic operated by Ms Marta Labuda under the name “PSYCHODNIA – OŚRODEK REHABILITACJI ZWIERZĄT MARTA LABUDA” is now the company LuxVet24 Platforma Weterynaryjna Sp. z o.o. Accordingly, the controller of your personal data processed by Ms MARTA LABUDA within the scope of her business is LuxVet24 Platforma Weterynaryjna Sp. z o.o. in Warsaw. 

Please also be advised Dr Marta Labuda will continue to manage the Psychodnia veterinary clinic and hold the position of Director of LUXVET24 Medical Programmes.

GDPR INFORMATION CLAUSE

In connection with the sharing of personal data, we inform you that:  

  1. In accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council of 27.04.2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and the repeal of Directive 95/46/EC (hereinafter “RODO”) in connection with the establishment of cooperation with the Company, respecting your privacy, we inform you that the controller of your personal data is LuxVet24 Platforma Weterynaryjna Sp. z o.o. (the “Company”) with its registered office in Warsaw, ul. Żurawia 8; for data protection issues, please contact: iod@grupaluxvet.pl ; 
  2. The purpose of the data processing is the proper performance of the contract for veterinary services, marketing of the Company’s own services and products, the assertion of claims, the sending of commercial information, the carrying out of analyses for the purposes of the Company’s activities and the fulfilment of legal obligations incumbent on the Company.  
  3. The legal basis for processing personal data is:
  4. contract for the provision of services (art. 6 (1)(b) GDPR), 
  5. Your consent to the processing of your data (art. 6(1)(a) GDPR),
  6. legal obligation (art. 6 (1)(c) GDPR), 
  7. legitimate interest of the Company (art. 6(1)(f) GDPR).
  8. The Company may transfer personal data to data recipients or processors of personal data on behalf of the Company to whom the Company outsources personal data processing services. Such entities shall process the data under contract with the Company only in accordance with the Company’s instructions and in compliance with the principles arising from the GDPR; the transfer of data may concern, in particular, entities providing animal treatment services to the Company.
  9. The Company does not transfer personal data to entities outside the European Union;

Period of retention of personal data – the Company retains personal data until the expiry of the statute of limitations for any claims arising from the contract for veterinary services, until an objection is made to the processing of the data, until the consent is withdrawn or the purpose for which the consent was given is fulfilled, whichever is sooner. 
  11. You have the following rights in relation to the processing of your personal data:

  13. The right to request the correction and rectification of personal data;
  14. The right to request the erasure or restriction of the processing of personal data;
  15. The right to object to the processing of your data on the grounds of your particular situation – in cases where the processing is based on the Company’s legitimate interest;
  16. The right to data portability, i.e. the right to receive the data in a structured, commonly used, machine-readable computer format;
  17. The right to lodge a complaint with the supervisory authority in charge of personal data protection, i.e. to the President of the Office for Personal Data Protection;
  18. To the extent that your personal data is processed on the basis of your consent, you have the right to withdraw it at any time, which will, however, not affect the lawfulness of the processing of your personal data before the withdrawal.

The Company collects information related to users’ browsing of the Website, in particular information such as IP address, date and time of visit to the Website. The Company does not link such information to your personal data and does not use it to identify you. 

The Company uses the information referred to above solely for statistical purposes and to improve the functioning of the Service.

Cookies are browser-supported textual information designed to facilitate web browsing. No personal data is collected or processed using cookies. Cookies allow the Website to be adapted to the individual settings and preferences of the User. The Company also uses third-party cookies to optimise marketing communications, count user traffic on the website and personalise content (e.g. advertising) from external sources. 

The user can deactivate the option to accept cookies in their browser at any time. However, this may result in difficulties in using the Website Services, some functions on the Website may not be available or may not function correctly. The Website uses Google Analytics, Google AdWords and other Google analytics tools, as well as Facebook Pixel, which help determine how Users use the Website in order to improve the Website.
